Please note: A referral is required for all specialty visits. The referral should be obtained from the member’s PCP. There is no specific Empire BlueCross BlueShield HealthPlus referral form. Referrals can be given on prescription or stationery.
We welcome provider referrals for patients who can benefit from additional education and care management support. Our case managers work collaboratively with providers to obtain input on care plan development, and provide telephonic and/or written updates regarding patient status and progress.
Please note: Members do not need a referral from their PCP to access behavioral health services; however, the PCP should actively engage in identifying the need for behavioral health services for their patients and should remain involved in treatment planning for all patients with behavioral health issues.
